Accidental release of lifeboat – life lost

Det Norske Veritas (DNV) has published a Casualty Information release on a lifeboat which dropped accidentally with six crewmen inside. One man died.

One contributing cause of this accident may have been that it was possible to latch the hydrostatic protection in the open position. Thus when the lifeboat had been launched and lifted out again, there may have been no verification that the hydrostatic protection engaged again preventing unintended release.

Lessons include checking release gear, and lowering/raising the lifeboat unmanned before this is done manned.
